Our friendly, knowledgeable staff works hard to make your visits to our office as smooth and stress-free as possible. We've provided helpful information about our office on this page, including directions, hours, insurance and billing, payment options and financing. For your convenience, the following forms can be downloaded on your computer and printed. Please be prepared to provide this information along with your current medical insurance information at the time of your appointment. If you have any questions regarding completing the forms, our staff will be happy to assist you on the day of your appointment.

Appointments

After Hours Access

You can always reach a doctor after-hours for urgent medical needs. Just call our main phone number 727-375-5437 and give a brief description of your concern to the answering service. They will page the on-call doctor who will return your call promptly.

To help make managing your child's healthcare needs easier we have a Patient Portal.  This provides you with access to your child's medical records and allows you to send non-urgent messages at any time day or night.  This means you can request forms, medication refills, non-urgent  appointments, and referrals online as well as view visit notes, imunizations, lab & x-ray results, and statements. You can also print your school immunization and physical forms right from the patient portal!

Meet-the-Doctor

Prospective Parents are welcome to call for a free Meet-the-Doctor appointment. You can check out our office and meet the doctor. Here you can learn more about what to expect and briefly address any concerns you may have in regards to your newborns care in the hospital and after coming home. New Moms please take note that Dr. Pham is a Board Certified International Lactation Consultant. Be sure to bring your questions with you!

Yes, we always reserve a limited number of appointments for same day urgent needs.

Walk-Ins and Add-Ons

In an effort to prevent long wait times and as a courtesy to all of our scheduled patients, we ask that you please call ahead for all appointments or to add-on other children that you want seen by the doctor.

Well Child Exams/School Physicals

All of our providers believe that preventive care is a vital aspect of maintaining your child's physical and emotional health and well-being. The American Academy of Pediatrics states these wellness exams should be performed at 2, 4, 6, 9, 12, 15, & 18 months and then once every year from 2-18 years.
